Transcript Request

Graduates: To obtain a copy of your high school transcript a “Transcript Request” form must be filled out and submitted with a payment of $5 per transcript. The form can be downloaded by clicking on the link below or it can be mailed, faxed or emailed to you upon request. If you wish to make payment by charge card there is a section on the form you can fill out and sign to do that, however the signed copy must be submitted in order to process the payment.

If you require an official transcript we will need the address of the school, scholarship or employer’s (company’s) address. Only unofficial transcripts can be mailed to personal addresses. Transcripts are mailed out within 2-3 business days after the Transcript Request form is submitted with payment.
